About one third of guys who are executed by hanging in this way – short drop – get an erection as they die, and some have orgasms, according to medical reports. Iran and Kuwait hang guys in multiple layers of clothing below the waist: diaper, tight under-trousers (like thermals), shorts, and then the victim's own trousers (Iran) or pyjamas (Kuwait). This is presumably to hide the victim's bodily functions and possible hard-on. In some Kuwait execution images you can see the multiple layers of clothing, including the top of a diaper or incontinence pad. There are some Iran hanging images where you can see that the guy has pissed or shat himself down his legs, but it is not very visible through his trousers, because of the layers. There are a couple of still shots of slow Iran hangings where the victim has a massive hard-on as he is taken down from the noose, and also a video of an Afghan hanging (no longer online) where you can see very clearly, as the guy turns slowly on the noose, that he has a huge hard-on beneath his robe.

ps I used to have a picture, in a book, of a Libyan hanging – which I've not found anywhere on the Internet – of four guys hanging. One had shat himself very visibly, but the guy in the centre was the amazing bit of the picture: there were two different stains on the front of his trousers. He had clearly pissed himself – it was running down both trouser legs – but there was a smaller, separate damp patch slightly higher up on his crotch, where there was also a visible bulge. His face was flushed a different colour and he was bearing his teeth. Basically the picture appeared to show him actually having an orgasm on the noose, after previously pissing himself. Sadly I lost the book.... I've been trying to find the picture for years.
