my guess is if they were to honestly want to torture a guys cock and balls they would probably start with an actual blowjob then probably while he was still erect and before he blow his wad they would sever/remove the cock and balls at the same time while still erect and then place both the severed erect penis and testicles into the guys mouth after they were cut off of his body. that would be my guess on how they would really want to be cruel in torturing a guy. remove his manhood while he's watching it being cut off.

they never seem to work on the package ( cock and balls ) I guess no torture in that. There was one pic I saw where they removed the package and stuffed it in the beheaded guys mouth. and then I wonder if they cut off the package before or after he was beheaded.
