Sounds like he died from a combination of severe dehydration, heat stroke, exhaustion and his heart rate was too high and he couldn't get enough oxygen intake.

Lack of training and preparations and not enough education in these areas?

emm, this case is a sudden death, i believe that is partly because of his lacking of sleep. this boy only sleep 4-5 hours a day to support his family
i think the maily reason for his death is the doctors' terrible knowledge, he was on emergency rescue for 20 minutes on the spot, but did not get his heart reworking
then he was sent to the ershadao division of traditional chinese hospital of guangdong province, where the doctors there saved him temporarily, but he died of failure of several organs because of the 20 minutes' heart failure and severe DIC next day
this boy is quite well trained in running and he planned to go bike riding after the marathon, but still, 10 km is a very long distance and there is not enough rescue on the spot
