The movie got it wrong. It should be a very dull pole, so that it can push aside blood vessels and organs. That is what allows the victim to live for up to a week. Here you can see that something internally got punctured and the condemned has died as the pole punctures his shoulder. And ideally the pole should exit the mouth.
