Looks like his executioners carved around his neck with a knife and almost, but didn't quite take the head completely off. Leaving a few tendons connected is just as effective as a clean cut, and also more stylish.

That might be his esophagus we see keeping body and head together. Is he hanging from the goal posts of a hockey rink? The athletes were probably back playing there the next day.

He may have. He may also have been killed by the opposing (and losing) team. Hockey and soccer are both rough sports, and players who aren't winning have been known to go berserk.
