Horrific helicopter accident from a few days ago in Russia involving two military dudes who both ended up beheaded and chopped up after mechanical failure caused their aging craft to fail and plummet into a lake.

They were so close to being okay. If the blade had just swiped a foot higher they'd have been able to swim to shore.
