Jay-R Marañon Serna was shot by unidentified gunmen. He was rushed to Cotabato Regional and Medical Center in Cotabato City, Philippines on October 15, 2016, but he was pronounced dead.

I guess that's the fatal bullet hole a few inches above his nipple, and the nipple does indeed have something mysterious projecting from it.
