Happened in Russia but I don't have more details.
 
On March 5, 35-year-old Andrei Yudin, at the Vostochny open-pit mine in the Iskitimsky district of the Novosibirsk region, pulled a flying caterpillar of a Komatsu career excavator and installed wooden bars for insurance. Unable to withstand the load, the bars broke and the repairman was crushed by a caterpillar of a multi-ton colossus.
As a result of the injuries, the man instantly died. As it turned out, this is not the first tragic incident on this section; 1.5 months ago, two workers died here as a result of the explosion of the BelAZ wheel.
