The first penis transplant performed worldwide took place at the Guangzhou General Hospital in China, in a 44-year-old recipient who had suffered the traumatic loss of his virile member in an accident. His donor was a 22-year-old brain-dead male patient whose parents agreed to the donation of their relative's penis, an event that was carried out in 2006. Unfortunately, on the 14th postoperative day, the transplanted penis had to be amputated due to severe psychological problems experienced by both the recipient and his wife; following histopathological analysis, it showed no signs of rejection.



The second penis transplant was performed in Cape Town, South Africa, at the Tygerberg Academic Hospital. The recipient was a 21-year-old patient who lost his penis 3 years prior to the transplant following a ritual circumcision that was complicated by gangrene of the proximal pedicle, preventing him from utilizing any of the functions of a penis. This generated a significant emotional impact, leading him to be considered eligible for the transplant. Taking into account the possible psychological effects associated with the transplant, such as psychosis or refusal of the allograft, the team was especially careful in selecting the transplant candidate, considering waitlist candidates who complied with treatment, clinic attendance, and who were psychologically suitable.

The donor was a 36-year-old brain-dead patient, from whom informed consent was obtained for multi-organ recovery, including the penis. The first erection was reported 3 months after the transplant, and the first satisfactory penetrative sexual intercourse occurred 5 weeks after the surgery, which were achieved with penile rehabilitation using tadalafil that was later gradually withdrawn. After 24 months of follow-up, the patient continued to feel well with no signs of rejection, reported satisfactory sexual intercourse with normal orgasm and ejaculations, mentioning that he felt the transplanted penis as his own, and even reported having impregnated his partner 6 months post-transplant. The most comforting aspect, and what most satisfies the medical effort, is that the patient stated that the most valuable outcome for him regarding his penis transplant was that he felt "happy," attributing this to recovering all the normal functions of the penis and not just one of them, such as the aesthetic component.



The third penis transplant was performed in Boston, Massachusetts, USA, in 2017 on a patient with a history of subtotal penectomy due to penile cancer. His donor was a brain-dead patient identified through the New England organ bank, demonstrating that vascularized tissue allotransplantation is a viable option for restoring normal external genital appearance, complete urinary function, and partial recovery of sensory and erectile function, which improve the patient's self-image.



The fourth penis transplant additionally included the penis, scrotum, and lower abdominal wall in a war veteran recipient who presented with severe injuries involving the traumatic loss of his penis due to an Improvised Explosive Device (IED). The patient's initial injury also included the amputation of both legs above the knees, and a significant loss of tissue from the lower abdominal wall, scrotum, and both testicles. One year after the transplant, the patient presents near-normal erections, the capacity for orgasm, and has recovered the satisfaction of feeling whole along with a better perception of his self-image.

 
You really have to wonder how strange it has to feel to have another person's body parts attached to your own body after the procedure. I wonder if they ever get over the fact that they have someone else's penis.
