It's probably real and extremely stupid. Bad enough that such objects can get stuck and have to be surgically extracted, sometimes they shatter. That guy is hemorrhaging and could die rather quickly if he doesn't get the bleeding stopped. If he lives, he may end up with a colostomy for the rest of his life.
Of course, if it wasn't for people like him, you tube might not be nearly as popular.
